    I didn't need additional documentation because i used my drivers license (without on the face of it) and then rescinded it afterwards.  i think someone else here had issues using a baptismal certificate.  you can also fill out a form with a witness that says who you say you are.  perhaps that is easiest?

      Got my new passport.  Filled out the DS-11 form highlighting the nonresident national area, and filling in my country as Oregon, no residence, no SSN, with the Sedm passport attachment.  I used my drivers license (no ssn on it), and my certificate of Live Birth from the State of Hawaii.  It took 10 weeks to process.

      I haven't seen any other new passports, but this one looks similar to my old one minus some new artwork.  the first page still says …

    “permit the citizen/national of the United States named herein” (shouldn't it say united States of America?)

    but the cover says Passport United States of America  (like my old one)

    my nationality is UNITED STATES OF AMERICA  (like my old one)

    there are no Endorsements or Amendments on page 27 (used to be page 24)

    am I correct in my assumption that this is a normal passport?  and until I can get the certified documents from my application in a few months, that it is impossible to distinguish myself from any ordinary US Citizen by this passport?

    or did I nullify my national status by using a Birth Certificate and Drivers License?  instead of a witness and baptism records?  The DS-11 form is the application, and its filled out correctly for a national, that should be all that matters is my train of thought.

    Hey Seraph,

    Glad to hear your success story!…I have one question…Did the Department of State request “additional documentation” to help them “prove” your identity”? If so, what did you send…..I know someone who tried using the same SEDM attachment, and they requested additional documentation (ie…school records, marraige license)…My friend didn't want to send anything that would link them to the old “SS#”…..Any advise is greatly appreciated….Thanks in advance!
