Hmmm Well I'm having a different experience altogether.

In December of 2004 I applied for a passport and used FG's modified DS-11.

I showed a BC and work ID and entered zeros for the SSN. The clerk was puzzled but accepted the app anyway since it met the criteria for acceptance.
