I help a friend of mine's co-worker with the attach document to stop his employer from sending in his paycheck per Notice of Levy with this attach document.

I also had him call the IRS and notify them he was sending in form 911 along with a copy of The Great IRS Hoax and a statement of his tax position on why he had not filed for the past 16 years. Which at that point they said not to bother and withdrew their Levy in demand of $22,000. The employer was notified there of shortly and confirmed the withdrawal. We still sent the IRS the documents anyway certified mail.
