42 Appendix 2 -- Collection Office Codes and Locations

This appendix contains the collection office codes, the corresponding name, and location. Use only your collection office code as listed in this appendix when completing the district/division portion of item 1 on Form OBD-230, Deposit Slip. To establish a new code provide the code, the location's name, address, and FTS phone number to the Debt Management Section (INS), Finance Staff, Justice Management Division. To establish more than one collection point within an already established district, add one alpha character to the existing code.

Forward the new code, location name, address, and FTS phone number to INS for incorporation into the automated system.
